A Comparison of Surgery & Percutaneous Drainage of Pyogenic Liver Abscess



Abstract
This study is based on clinical analysis of 77 patients with pyogenic liver abscess admitted to the Department of Surgery, Kyungpook University from 1983 to December 1993.
@ES The results were as follows:
@EN 1) Pyogenic liver abscess was more often involved the right lobe(61.0%) than the left(26.0%) and a single abscess was enconuntered more often than multiple abscesses in 1.9:1 ratio.
2) The most common etiology was cryptogenic (44.2%), followed by pathology in the hepatobiliary tract(40.3%).
3) In the bacteriologic study, we obtained positive cultures in 71.4%, and E. Coli was the most frequently cultured organism.
4) Post-treatment complications were occured in 17 cases(38.6%) with surgery and 14 cases (42.4%) with percutaneous drainage.
5) The outcome of surgery was similar to that of percutaneous drainage with single pyogenic abscess.
6) The outcome of surgery was more favorable in that of percutaneous drainage with multiple pyogenic abscesses.
7) Rescent trends in the treatment of pyogenic liver abscess has been percutaneous drainage.
8) Overall mortality rate was 7.8%: that of single pyogenic abscess was 6%, multiple pyogenic abscess was 11%.
